The custodial parent agrees to forgive arrears prior to 2004.
All payments have been made beginning with the order starting 2004 to the present.
What type of letter or form can does the custodial parent need in order to forgive the arrears?

I suggest that you contact the Florida Department of Revenue, and make this request to them in a letter.
However, without a court order to forgive the payments in arrears, and a very compelling reason, it is unlikely that they will be forgiven.
Child support is for the benefit of the children, and generally may not be modified even upon the consent of both parties.
The courts need to ensure that the children's best interests are protected.
